This 30 year-old man presented with progressive painless nasal obstruction and widening of the nose of two weeks' duration. Upon questioning, he denied having had surgery, but gave a vague history of nasal trauma, while playing soccer.
On physical examination, the septum was ballooned, fluctuant and filled both nasal cavities.
Coronal CT scan image showing expansile swelling of the septum. Notice the complete bilateral nasal airway obstruction.
On incision and drainage, thick yellow pus was recovered.
A Penrose drain was left inside the abscess cavity and Doyle nasal splints were placed bilaterally to compress the septum.
